I give permission to New Millennium Foundation 2020 that my minor child listed in this release form may be photographed while participating in the programs offered, on field trips, or other activities. I understand that these photographs may be used in the promotion of the services offered, either in print, or on the internet for any legal use, including but not limited to publicity, copyright purposes, illustrations, advertising, and web content.

I hereby release and hold harmless New Millennium Foundation 2020 from any reasonable expectation of privacy or confidentiality for myself and for the minor child associated with the images. Further, I
attest that I am the parent or legal guardian of the child and that I have full authority to consent and authorize New Millennium Foundation 2020 to use their likeness.

I further acknowledge that participation is voluntary and that neither I, the minor child will receive financial compensation of any type associated with the publication of these photographs or participation in New Millennium Foundation 2020 marketing materials or other New Millennium Foundation 2020 publications. I acknowledge and agree that publication of said photographs confers no rights of ownership or royalties whatsoever.

I hereby release New Millennium Foundation 2020, its associates, and any third parties involved in the creation or publication of New Millennium Foundation 2020 publications, from liability for any claims by me or any third party in connection with the participation of the minor child.
